VIP Replacement Certificate - Louisiana
WARNING!! This a payment form for requesting a DUPLICATE CERTIFICATE FOR A MADD VICTIM IMPACT PANEL. If you have NOT attended a MADD VIP, you will need to return to maddvip.org to register for an upcoming panel. MADD VIP fees are non-refundable. If MADD staff cannot verify attendance, we may call or email for additional information. If you did not attend a MADD Victim Impact Panel, a refund will not be processed. To receive a duplicate MADD Victim Impact Panel Certificate for a panel completed in LA, please complete this payment and request form.
You will be emailed or mailed a duplicate certificate within 3-5 business days. Our staff will need to verify you attended and paid for a past panel before we can issue a duplicate certificate.
WARNING! This payment form is ONLY for MADD Victim Impact Panels completed in LA within the past 12 months.
If six months has passed and you have misplaced your certificate, you have the option to either attend another VIP class or take the online course.
