Maryland VIP Replacement Certificate WARNING!! This a payment form for requesting a DUPLICATE CERTIFICATE FOR A MADD VICTIM IMPACT PANEL. If you have NOT attended a MADD VIP, you will need to return to maddvip.org to register for an upcoming panel. MADD VIP fees are non-refundable. If MADD staff cannot verify attendance, we may call or email for additional information. If you did not attend a MADD Victim Impact Panel, a refund will not be processed. Request to Receive a Duplicate MADD Victim Impact Panel (VIP) Certificate To receive a duplicate MADD Victim Impact Panel Certificate for a panel completed in Maryland, please complete this payment and request form. You will be emailed or mailed a duplicate certificate within 3-5 business days. Our staff will need to verify you attended and paid for a past panel before we can issue a duplicate certificate, please visit maddvip.org to attend a new MADD Victim Impact Panel class. If your VIP was within two years you can receive a duplicate Certificate, if not please register for a new class. For questions, email MD.State@madd.org WARNING! Arriving early is recommended as walk ins are on a first come, first serve basis. The only form of payment accepted as a walk in is a $65 money order. Please have your photo identification and court paperwork ready when you arrive.
